Immunization Action Plan – This program is funded through a grant from the Ohio Department of Health for the purpose of increasing age appropriate immunization levels in children under the age of 24 months. The grant provides money for service delivery, public information, education to care providers, and assessment of provider’s vaccine administration successes. A registered nurse serves as the program coordinator.
